چکیده

Introduction Wide complex tachycardias have a broad differential diagnosis including ventricular tachycardia (VT), antidromic reciprocating tachycardia (ART), supraventricular tachycardia (SVT) with aberrancy, atrial tachycardia (AT) or atrioventricular nodal reentry tachycardia (AVNRT) with a bystander accessory pathway, or unusual tachycardias involving nodoventricular or atriofascicular pathways. Diagnostic criteria for differentiating VT from SVT with aberrancy have been developed, all having similar diagnostic accuracy, but are limited in the ability to distinguish VT from pre-excited tachycardias because morphology on surface electrocardiogram (ECG) is identical to VT originating from the basilar segment of the ventricle. In this case of wide complex tachycardia, we use observations of intracardiac electrograms during tachycardia as well as a single atrial extrastimulus to make a diagnosis in our patient.

Electrophysiologic demonstration of an atriofascicular accessory pathway.

Evidence of an atriofascicular accessory pathway has not been reported previously. In the patient presented in this case report, an electrophysiology study demonstrated a constant left bundle branch block QRS configuration despite varying degrees of ventricular preexcitation, in association with a constant artrioventricular interval during incremental atrial pacing and programmed atrial stimula...

Ventricular fibrillation during electrical cardioversion of pre-excited atrial fibrillation.

The Wolff-Parkinson-White syndrome can rarely present with pre-excited atrial fibrillation. In this condition the short refractory period of the accessory pathway can lead to rapid atrioventricular conduction. There is then a danger that at high heart rates the irregular broad complex tachycardia that results can deteriorate into ventricular fibrillation. The initial management of patients pres...

Limitation of tachycardia zone resulting from longitudinal dissociation of the atrioventricular node in concealed pre-excitation.

Two cases with a concealed left-sided accessory atrioventricular bypass tract are described. In both, functional longitudinal dissociation of the atrioventricular node narrowed the range of atrial premature beat coupling intervals which could initiate re-entry using the accessory pathway. In case 1 early premature atrial beats were followed by an atrioventricular nodal re-entrant echo. The atri...

Supraventricular tachycardia. ECG diagnosis and anatomy.

This paper reviews the anatomical substrates responsible for the induction and maintenance of supraventricular tachycardia and discusses the ECG findings associated with these tachycardias. The normal anatomy of the supraventricular conducting system, particularly within the atria, is complex with conduction proceeding along preferential pathway, which are in turn determined in part by the anis...

Atypical atrioventricular node reciprocating tachycardia masquerading as tachycardia using a left-sided accessory pathway.

OBJECTIVES The study was performed to document that atrioventricular node reciprocating tachycardia (AVNRT) can be associated with eccentric retrograde left-sided activation, masquerading as tachycardia using a left accessory pathway. BACKGROUND The eccentric retrograde left-sided activation during tachycardia is thought to be diagnostic of the presence of a left free wall accessory pathway. ...
